ACSM Clinical Exercise Physiologist: CEP Requirements, Exam and 2027 Rule

The ACSM clinical exercise physiologist credential, called the ACSM-CEP, is the American College of Sports Medicine’s certification for working with patients who have chronic disease. You qualify with a master’s in clinical exercise physiology and 600 clinical hours, or a bachelor’s in exercise science and 1,200 hours. The exam costs $460, or $350 for ACSM members, and a new degree rule starts August 15, 2027.

What Does an ACSM Clinical Exercise Physiologist Do?

ACSM says its certified clinical exercise physiologists help patients gain long-term physical, social and economic independence through individualized education, behavior change, and primary and secondary prevention. In plain terms, you work with people who are sick or recovering, not people who want a personal best.

The clinical work ACSM lists includes cardiac and pulmonary rehabilitation, exercise testing, exercise prescription, electrocardiography (ECG), patient education, and managing heart, lung and metabolic disease. The My Next Move for Veterans career site lists the ACSM-CEP as accredited by the National Commission for Certifying Agencies (NCCA). That matters to some employers, so it’s worth knowing before you choose between this credential and a personal trainer certification.

What Are the ACSM CEP Requirements?

ACSM sets two pathways, and the degree you already hold decides which one you take. Both need a degree from a regionally accredited school, documented clinical hours, and a CPR card that includes a hands-on skills check.

You don’t have to wait for your diploma. ACSM lets you apply and test in the final semester of your degree, as long as you upload a transcript showing active enrollment.

Now the part that surprises people. Every ACSM-CEP application gets a 100% audit by ACSM’s CEP External Review Board, which checks your transcript and your clinical documentation. The ACSM-EP audits about 10% of applicants. Keep your hour log clean from day one.

ACSM also wants your program to include formal preparation in three areas: clinical exercise testing and prescription, advanced exercise physiology, and clinical exercise physiology and pathophysiology. Its detailed list names ECG interpretation and the pharmacology of commonly prescribed drugs, so read a program’s course titles, not just its name.

What Counts as Clinical Hours?

Clinical hours must come from a clinical setting. ACSM’s candidate handbook says they can come from internships, volunteer hours or work experience, so a job as a cardiac rehab technician can count.

Here’s what doesn’t count: observation-only hours, non-clinical experience, hours with athletes or healthy clients, and personal training. Virtual hands-on counseling is accepted, but no more than 50% of your hours can be virtual.

The handbook also gives a recommended split by condition. ACSM calls it a recommendation, not a quota, and it shows what reviewers expect to see:

Do the math on your own schedule. At 40 hours a week, 600 hours takes 15 weeks and 1,200 hours takes 30. At 20 hours a week, double both.

What Changes for ACSM CEP Requirements on August 15, 2027?

The date your degree is awarded decides which rules apply. If you graduate before August 15, 2027, today’s requirements cover you. If you graduate on or after it, your degree must come from a program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP), or from one that has formally started the process.

For the CEP, that means a CAAHEP-accredited Exercise Physiology program for the master’s pathway, or a CAAHEP-accredited Exercise Science program for the bachelor’s pathway. ACSM’s programmatic accreditation page states the rule.

“Started the process” has two meanings. One is a free formal inquiry to CAAHEP. The other is a Request for Accreditation Services (RAS), which asks CAAHEP to review the program. In May 2026, ACSM confirmed that graduates of in-process programs stay eligible if they provide proof of the program’s status. It also said the board will reassess the rule in 2029.

How many programs qualify today? The CAAHEP list we pulled on September 29, 2026 shows 28 U.S. master’s programs in Exercise Physiology with continuing or initial accreditation. Eighteen of them carry the Clinical concentration.

Only two of those 18 are on our online list: Lebanon Valley College and University of Mary. Both keep required in-person components, which we cover below.

On the bachelor’s side, Northeastern State University’s online BS in Health and Human Performance appears on CAAHEP’s Exercise Science list at initial accreditation. Franklin University and Lander University each show a Letter of Review for a Master of Science in Exercise Science. Ask both schools whether that covers the online program you’d enroll in.

How Does the ACSM-CEP Exam Work and What Does It Cost?

The ACSM-CEP exam has 115 questions. One hundred are scored and 15 are unscored, and you get 210 minutes. ACSM converts your raw score to a scale of 200 to 800 and sets the passing point at 550. You see your result as soon as you submit.

The handbook publishes the weight of each area, so you can split your study time to match:

The fees are $460 for non-members and $350 for ACSM members, with a $50 non-refundable application fee inside that price. A retest costs $235. Members save $110, so price out a membership before you register.

If you fail, you wait at least 15 days to retest, and you can sit up to four times in 12 months. After four misses, you wait 12 months.

How hard is it? ACSM’s public pages we could read don’t give a pass rate. The University of Mary, which is school-reported data and not ACSM’s, says its graduates had an 80% cumulative ACSM-CEP pass rate from 2015 to 2025, against a 65% national first-take rate in 2024. ACSM recommends two texts, ACSM’s Clinical Exercise Physiology and ACSM’s Guidelines for Exercise Testing and Prescription, plus a third review book and an online prep course. It also says you can take the exam without buying any prep materials.

Can You Earn the ACSM-CEP Through an Online Degree?

Yes, for the coursework, and no for the hours. Every clinical exercise physiology program on our list puts a supervised internship, on-campus labs, or both, in front of you. The online part is the classroom.

Lebanon Valley is the closest fit to ACSM’s language. It’s confirmed on the CAAHEP list, its 600-hour internship matches the master’s pathway exactly, and its courses include ECG Interpretation I and II and Pharmacology. At 33 credits and $789 per credit, tuition works out to $26,037 before aid, and the GRE isn’t required.

University of Mary is also CAAHEP-confirmed. It admits one cohort a year and starts September 6, 2027. It adds a $55 per credit technology fee, and the on-site lab weeks mean you’ll travel to Bismarck, North Dakota, every semester.

UW-River Falls has the lowest cost per credit on this table. It’s 34 credits at $567, or $19,278, and includes Electrocardiography and Cardiopulmonary Rehabilitation courses. We didn’t find CAAHEP status for it, so ask before you count on the 2027 rule.

Tiffin has the most flexibility and the biggest gap. Its internship is 150 hours, and ACSM’s master’s pathway asks for 600, so you’d still need 450 more (600 minus 150) from work or volunteering. Franklin doesn’t state its internship hours, and its availability varies by state.

Fourteen programs on our list mention the ACSM-CEP on their own pages. The wording ranges from “designed to prepare you” to a graduate saying they passed. Read the exact sentence before you treat it as exam preparation.

Should You Take the Bachelor’s Pathway or the Master’s Pathway?

If you already hold a bachelor’s in exercise science, the bachelor’s pathway costs you no extra tuition. It costs you hours: 1,200 instead of 600. If you’re working in a hospital or rehab clinic, those hours may build up on the job.

The master’s pathway halves the hours, but it adds a graduate degree. The median online master’s on our list costs $610 per credit and $20,200 in total tuition, and most programs run one to two years. That’s a real tradeoff, especially if you’d have to leave work to finish an internship.

A master’s makes more sense if your bachelor’s is in a different field, you want to move into a clinical lead role, or your target employer asks for it. ACSM-CEP vs ACSM-EP: Which Credential Fits You?

Both credentials share a fee and a renewal cycle. They differ in who you serve and what you must prove. Our ACSM exercise physiologist guide covers the ACSM-EP in full.

The short version: pick the ACSM-EP if you want to prescribe exercise for generally healthy people. Pick the ACSM-CEP if your goal is working alongside physicians, nurses and rehab teams with patients who have disease.

How Much Do Clinical Exercise Physiologists Earn?

BLS doesn’t publish a separate line for clinical exercise physiologists. The closest category is Exercise Physiologists, and the May 2025 OEWS data puts the national median at $59,460 a year and the mean at $60,940. The lowest 10% earn $42,760 or less, and the highest 10% earn $80,560 or more.

BLS counted 8,560 exercise physiologists nationally. By state, Washington had the highest mean at $81,870, followed by Minnesota at $72,750.

O*NET lists growth for exercise physiologists from 2024 to 2034 as much faster than average and projects 1,700 openings. Those are projections, and no credential guarantees you a job or a salary.

Do Any States License Clinical Exercise Physiologists?

Louisiana does. The Louisiana State Board of Medical Examiners runs a clinical exercise physiologist licensing page with an application, laws and rules. Its application instructions list a master’s degree and ACSM certification among the requirements.

Those instructions still use older ACSM credential titles, so ask the board how it treats the ACSM-CEP today. If Louisiana is in your plans, get the board’s current rules before you choose a program, because the degree language matters.

Rules elsewhere can differ, and they change. Check your own state’s board before you count on the credential alone.

How Do You Renew the ACSM-CEP?

You renew every three years. ACSM’s recertification page asks for 60 continuing education credits (CECs), a current CPR credential, and a $70 fee for the ACSM-CEP.

You can earn up to five CECs a year from CPR, AED and first aid courses. ACSM audits 10% of renewals. If yours is picked, you have 30 days to send documentation, or the credential goes inactive. A late renewal costs $80 with a six-month grace period, and reinstatement costs $225.

What Are the Downsides of Aiming at the ACSM-CEP?

The credential is respected, but the path has friction. Here’s what to weigh.

The hours are the bottleneck. Finding 600 or 1,200 hours in a clinical setting takes planning, and observation doesn’t count. Start asking hospitals and rehab programs about placements before you apply to a school.

Online options are thin. Only two online master’s programs on our list are confirmed on the CAAHEP list, and both need campus visits. If you can’t travel, your choices narrow fast.

The pay is modest for the training. A $59,460 median for exercise physiologists, after a graduate degree, is a real consideration if you’re financing school. Run the numbers before you borrow.

The rules are still moving. ACSM has already widened the 2027 pathway once and says it will look at the rule again in 2029. Check the programmatic accreditation page again before you enroll.

What Should You Ask an Admissions Office About the ACSM-CEP?

Send these before you apply, and keep the answers in writing:

• Is this exact program on the CAAHEP list, or has it filed a formal inquiry or Request for Accreditation Services? Can you show me the confirmation?

• How many clinical hours does the degree itself include, and where do the rest come from?

• Who finds and approves my clinical site if I live far from campus?

• Do the syllabi cover ECG interpretation, exercise testing and the pharmacology of common drugs?

• Will my graduation date fall before or after August 15, 2027?

• How many graduates sat the ACSM-CEP last year, and how many passed?

An answer like “we prepare you for the ACSM-CEP” that never mentions CAAHEP or clinical hours tells you something. Ask again, and check the CAAHEP list yourself.
